 Registration Instructions for Coaches
The following TEAM forms must be included in your submission:
a) Official "State Roster," preferably in alphabetical order.

The following PLAYER forms must be filled out by and submitted on behalf of each player:
a) Player Membership Form.
b) Medical Release Form.
c) Sportsmanship Pledge.
d) A copy of the players' birth certificate (highlight in yellow the name and birthdate).
e) Signed player pass with photo (on photo paper, not copy paper) attached.

The following VOLUNTEER COACH forms must be filled out by each volunteer coach and included in your submission:
a) Sportsmanship Pledge.
b) Coach Membership Form.
c) Kidsafe Disclosure Form .
d) Signed coach pass with photo (on photo paper, not copy paper) glued.
e) Copy of coaching license.

The following PAID COACH info must be submitted for any professional trainers/coaches who will be on your state roster:
a) copy of both sides of their Coach card aka "non-volunteer pass" that the trainer will obtain directly from the state and give to you.

General Instructions that must be carefully adhered to:
1) The passes can have no cross-outs, white-outs, corrections, etc. If you make an error, you will have to use a new pass.
2) The State Roster can have no cross-outs. However, you may use white-out, but be sure to white-out all four pages (don't forget the three carbon copies).
3) The player name on the pass must match exactly the name on the State Roster AND on the birth certificate. No nicknames. No initials.
4) Players must sign the card with the full name (for example, "Samuel" not "Sam"), and in script (except for the very young kids who can't write in script yet).
5) Coach and parents should never sign for the player. Player must sign herself.
6) Numbers you will need for various forms and Roster: DC #47 League #12 Club# 9164
7) All forms must be separated by type, in the same order as the names appear on the roster. For example, all medical forms bunched together in order with a clip; then the bunch of Player Forms in the same order; then the Passes, etc.
8) Let me know via email when you have dropped off the package. You are welcome to make an appointment to go over the package together.
9) The club fees ($460 per player in 2008-09) must be delivered by you to the club treasurer, prior to me releasing the player passes to you.
10) Photos must be glued to the player pass when submitted.
11) All Medical Releases must be notarized. Please note that an attorney may notarize these. Important: a family member should not be the notary or attorney.
